This spreadsheet is provided as an example of the format that should to be used prior to referencing it within the python code. The attachment is an example of the fields and formats of information required to be in a spreadsheet before using the python code to format it into a Survey 123 survey. The field names need to stay the same and in the same order in the spreadsheet (don't capitalize names, don't move columns, etc).
